The vest is available in both ladies and men's styles. Please view the size charts, which are included in the ordering form. Note: the UCI Health-branded vests cannot be customized with names and/or credentials.
Please do not forward this message, as eligibility is restricted to those receiving this email. Only one vest may be be ordered per co-worker. The database will be checked for duplications.
Your vest will ship to the mailing address you enter in this order form. Please use an address that you trust for receiving deliveries. UCI is not responsible for vests stolen from the delivery address following proof of delivery by contracted carrier.
We are unable to send out duplicate gifts or process exchanges/returns, so please be sure to double check your style, sizing, and mailing address for typos BEFORE you submit the form. Once submitted, all orders are considered final and the form cannot be edited.
You must enter your UCI email address (ending in @hs.uci.edu) in the submission form. Any orders containing personal email addresses will be rejected from the system.
The deadline to place your order is June 30, 2024. We cannot accept orders past this date.
Please allow 4-6 weeks after your order submission for delivery.
